According to the JTS user manual, infantry units that fail a morale check from offensive fire rout while those that fail a morale check from defensive fire are disrupted. In addition to casualties, those are the only consequences of fire for an infantry unit according to the manual.

However I have noticed several times that offensive fire has caused units to become disrupted without routing and was not caused by adjacent units routing or from melee. So there is some way for units to become disrupted from offensive fire that's not explained in the manual.

Does anyone know how that works?

I think I answered my own question. It seems that the manual does not tell the whole story regarding the effects of offensive fire.

For offensive fire: Units that fail a morale check rout. Units that pass a morale check become disrupted. That second part is not in the manual.

This also explains why units adjacent to or in the same hex as a unit that routs automatically become disrupted if they don't rout themselves. It's because they're all automatically forced to make morale checks.
