Gentlemen, are the optional rules set anew for every battle in a campaign game, or are they set once and for all with the first battle of the campaign?

Every battle, within the campaign are set seperately. Essentially, each campaign game is unique, but linked to the campaign.

Yes, Ernie is correct. But if you notice, the game remembers the last set of optional rules you used on a game you start...so if the same player starts all the battles you essentially only have to set them once.
